The doctor sends the request for payment form to the carrier and bills the beneficiary only for the part of the bill that medicare doesn’t cover. , With the second method the. same request for pay ment form is used, but the pa tient himself sends the claim to the carrier after paying the doctor’s bill. When the beneficiary is go ing to send the claim to the medicare carrier himself, he fills out the first part of the request for payment form-and then has the doctor complete the second part of the form showing the services and the charges to the patient.
