With the second method

to the carrier after paying
the doctor’s bill.
When the beneficiary is go
ing to send the claim to the
medicare carrier himself, he
fills out the first part of the
request for payment form-and
then has the doctor complete
the second part of the form
showing the services and the
charges to the patient. If the
doctor dloes not. fill out the
second part of the form, how
ever, the beneficiary must
enclose an itemized receipted
bill.
