BBA?? SUBGKB?. .Flnhrei's Remarkable Operations at (telle rae Uocpltai. ?fae lVt H'orfrf, -Vry 23. Professor Flubrer of the Twiting sur gical slafTof Bellevue Hos, ital yesterday presented to a large gathering of medi cal gentlemen, among whom wire Trois. Lewis A. Sayre, A. Post and Charles ilittendorf, bi?. Anderten, Putnam. Jae i lu and others, thc mo?t remarkable case of successful curg'wal operation bf thc brain that ever came under tue obser vation of a -urgeon and which ?till remains uuexplaiiud from the fact tba*, the patient recovered and is at present enioying the best of health. The case has no paraliel in the annals of medical literature and until it present ed itself the roost eclectic surgeon would have doubted the poisiblity of introduc ing the knife into the brain without fatal results. Tbe patient, Bruno Koorr, who yester day eat m the amphith??tre of the hos pital in the centre of a group of ioten-ejy interested physicians, who carefully ex amined his head, which was a inti* of scarp, showing where the bone was cut out, has a remarkable history, lie is a well-formed young man, and were it not for bis much-chopped up forehead, would ' be quite good lookitig. He appear? to be of a very affectionate dUpositipn, which is the cause of bis strange wound. Ou the evening of January ?3 last, as printed in the yforhl. at the time, he visited his sweetheart and asked her to marry bim, j \ but abe declined; be pressed his snit, but ?ras ditly refused, upon which he pulled out a revolver and fired a bullet into hi* brain. The ball entered the forehead, about cn inch and a half above the eyes, directly in the centre. He fell ! t to the ground with blood gushing pro- I ?j fundy from the wound. He was supposed ' t > bc dear!, but nevertheless he was re- j n mored to Bellevue Hospital. Upon arriv- I . in? at that placo thc junior surgeons j t were surprised to find thal he was ?stilt | ^ alive. Tn rough the r.ight tiny tat by his bedside, intent upon making observa tions SA to bow long hs could really live, applying only such simple remedies as wonld ease the Ies1 moments of a dying man. Bul Lucy miscalculated, for iu the morning they found him still alive, al though unconscious Afraid to do any thing upon their own responsibility, they telegraphed the tithing surgeon, Prof, Flubrer, at day brea'?, and bc came at once in response to the summons, hardly supposing ne could do anything for the patient. From certain peculiar surround* inga, however, he was led to undertake a moat hazardous operation, which he described to his listeners yesterday while r'auuing at the side of thc man whose life be bad saved. Finding the puLMiou of the patient growing weaker and life slowly ebbing, ne took the most desperate chances and resolved to operate at once. Feeling as sured the braiu was wouuded, from the small particles of its substanre thal oozed through the hole made by the bul let, he boldly set to work, and, thinking that be could do no more than apply tho last measuren, he dissected off the flesh aronnd the wound clean to the bone, holding back the flaps with pins, and then cut out a disc of the skull about the sizo of a dime with a trephine above the bullet hole, joining the two by chiselling away the bridge intervening. On p.t temptiug to remove the particles of bono which had been driven upon the brain matter by the bullet in its passage and bad set up a dangerous condition by their pressure, he met with a profuse hemorrhage from one of tho largo arteries, which was cut across its gaffed edges scarcely to be seen. Two attempts to catch it up proved futile, and realizing that death from loss of blond under the circumstance* was a matter of moment he introduced his floger into the wound and pressed the artery against the hone and for a time became master of the situ* atina. The exertion tired bim soon and he instructed big assistant to relieve bim .while be again attempted to catch the end. -After five trials be was successful and placed a ligature of catgut around it. Tho little vessel, however, was tricky, 'and the ligature slipped off, leaving mat ters worse than bet?re. Again be tried it and at last overcome the obstacle by tying it tightly. Next be introduced a bullet lipped probe, which descended by its own weight, about five inchea into the brain, when he stopped short and held it in place. Carefully studying the angle in which it entered be marked a point at tho back of tbe head where it would if further pushed come out, and there cut off three flaps aroundit and sawed through the bone, exposing the brain coverings in the rear. There he cut tbrougb and met with a mass of dark, half-clotted blood mixed with particles of brain which floated about. This was carefully washed out. He pissed in a probe smaller than the first and felt tbe bullet, lodged aa it was in the brain. ' With delicate foreeeps it was pul'tcd out tbrougb. the opening be hind. Thc operation occupied four hours. A rubber tube was drawn into the wound tbrougb the channel cut by the bullet by means of which the septic matter j waa allowed to flow. It remained ( there for over a month until recovery had so far advanced that the patient ?roa declared out of danger f ' the surprise of all and in conflict whh the established roles in atsrgeryv' Prof.iFlahrex enlightened bis1 hearers with the treatment after the trper-: . nt i on and presented a chart shaw in g tba variations of the pulse, temperature-ant! respirations wbichjihe obsede*altoiOg the progresa of the case.. He ?"id frc u ?tresa opon the difficulty h? hftdTn draw ing ont several horse hairs which ho in trounced around the drainage tub? to prevent its adhering to tbe ?ralla of the wound. After the most critical stages had . passed it waa found that Knot? bad lost his memory, powers of speech sod wa? partially paralyzed, but ho hos recovered all hui faculties and pronounced himself as safe and sound as a dollar when ques tioned by the. doctors yesterday. .Dbe nearest approach of Knorr'a case Li one reported lj Prof. Dotros?d cf this city, who about fifty/years ago tr?phfn?d the skull of a patient suffering from ah abacera of tbe brain, which be located, and, by J trenbiuing the skull, exposed tbs brain j and opened the abscess by cutting into - it, but the patient only survived a few .months, when he died from a recurrence . of the disease.
