Answer — Ask for a special post card form at your nearest social security office. It is Form OAR-TOM (“Request lor Statement of Earnings**). If the repent sent you from Social Se curity Administration headquar ters does not agree with your own records, get in tuoch with your sodal security office to correct the error. Question—I receive interest income from my savings ac count at the bank, and they have asked me to get a social security number for identifica tion. I already have a social security number; do I have to get another one for the bank? Answer—No. Only one num ber is necessary for both pur poses. Your social security number is also used as a tax payer’s identification number for Internal Revenue Service purposes. Question—What is the total amount of social security tax which should have been with held from my wages in 1964? Answer—The employer’s por tion of the social security tax for 1964 is 3% per cent of wages up to $4800, which would result in a maximum of $1741 being withheld. 4 Question—I worked for two employers during 1964 and had total earnings of $6600. I just rsceived my W-3 forms and both employers withheld social security taxes on my total wag es. I thought the social secur ity tax was paid on only the first $4800 earned each year. Why did I have to pay on $6,- 800? Answer—Social Security taxes are due on only the first $4,- 800. However, each employer must withhold the tax on wages up to $4800. You probably didn’t earn $4800 from either employ er and each withheld the tax on your total earnings. Since you have paid the tax on a total of $6600, a refund is due. This may be obtained when you file your 1964 income tax return, or you may file a special clam with the Internal Revenue Ser vice for a refund of the over payment. Question—My emloyer insists that I apply for a new social security account number card. I last aay original card hut my number is oa my old tax re turns. I gave him this number, but he says to get a new card. Is this really necessary? Answer — Your employer Is right. The number on your tax return is probably correct, but a mistake could have been made when you copied it. The only way your employer can be sure he has the correct num ber is to take it directly from your social security card. You can get an application for a du plicate card at your social se curity office. The records will be checked, and you wUl re ceive a new card with your original numbef on it. Question—I have two social security account numbers. Should I do anything about this? Answer—Yes. Ask the Social Security Administration to com bine both accounts and furnish you a statement of the combin ed accounts.
