To provide extra service with your coverage, a new prog ram, called PROVIEW, was added to your benefits plan. PROVIEW reviews the medic al necessity of inpatient care before you go to the hospital — a process called pre admission review. PROVIEW Can Help Reduce Your Medical Expenses The PROVIEW program is managed by ex perienced medical professionals who can help you get the best medical treatment pos sible while helping to cut your medical costs. If your doctor suggests that you go to the hospital, PROVIEW may lower your costs for medical care. If you use PROVIEW, you are paid the regular benefit for hospital services covered under your medical benefits plan. If you do not use PROVIEW, your benefits may be re duced. PROVIEW Helps You Before You Go To The Hospital The PROVIEW staff works with your doctor before you are admitted to the hospital for any non-emergency procedure. Here is how PROVIEW works: •You inform your doctor, not person in emergency room or hospital. Before you or a covered family member are admitted to the hospital, tell your doctor you are a partici pant in the PROVIEW program. •Your doctor calls PROVIEW. Ask your doctor to call PROVIEW and give the follow ing information: —Name, address, telephone number, birth date and Social Security number of patient and employee —Your employer’s name and address —Admitting diagnosis —Patient’s signs and symptoms —Lab test results and x-ray results —Scheduled procedure date —Scheduled admission date —Projected length of stay —Hospital name, address and telephone number. Your doctor can call PROVIEW toll-free Monday through Friday between 8 a.m. and 4 p.m. in all time zones. The toll-free num ber is 1-800-621-4309. Questions and Answers Must I use PROVIEW? If you use PROVIEW you will qualify for regular coverage under your benefits plan. If you do not use PROVIEW, your benefits may be reduced. Who is covered by PROVIEW? The PROVIEW program includes you, your spouse and your dependents who are co vered under your group health benefits plan. PROVIEW does not cover Worker’s Compen sation patients. PROVIEW may cover Medi care patients if they are covered by a group health plan which is primarily responsible for their benefit coverage. Is my doctor the only one who can call PROVIEW? Your doctor or your doctor’s representative should call PROVIEW with the necessary in formation. Your doctor's representative is any employee of your doctor who is approved to give out this information. What if emergency treatment is neces sary? If you are admitted to the hospital for emergency treatment, PROVIEW must be notified within two working days of the admission in order to qualify for regular cov erage under your benefits plan. Your doctor or doctor’s representative can call the toll- free number, 1-800-621-4309, or in Ten nessee, 1-800-228-6016. What if hospital care is proposed when the patient is away from home? PROVIEW is available nationwide. PRO VIEW may be contacted by doctors across the United States by calling this toll-free number, 1-800-621-4309, or in Tennes see, 1-800-228-6016. What about maternity admissions? Your doctor may call PROVIEW within 30 days of the expected date of delivery. However, if hospitalization is required be fore this date and PROVIEW has not been notified, the doctor or doctor’s representa tive should call PROVIEW within two work ing days of the admission. Should PROVIEW be notified for newborn admissions? If a newborn is unable to leave the hospital when the mother is discharged, the baby’s doctor should call PROVIEW and provide the diagnosis and other necessary information so the review process can begin.
