You must present a form 406, which can be obtained only from your county treasurer's office. The second innovation in issuing vehicle licenses is the personalized license that is being offered to anyone who is willing to pay 15 dollars extra for a set. The deadline for requesting this type of license has been changed from November 1 to December 1.
