is not always on checks and receipts. Submitted
information must have name of the
provider of service, name of patient; name of
subscriber; date of service: brief description
of service; and amount charged for service.
Drug forms contain information on date of
purchase, prescription number, name of
drug, quantity, charge and pharmacist's
signature. All must be filled out.
