Q. If I am on Leave of Absence do
I have to keep premiums paid up, and
if so, where do I pay them?
A. Premiums must be kept up to
date while you're on Leave of Absence
and your payments must be paid weekly
at the Payroll Department at Clinton
Mills Office.
Q. If husband and wife are both
employed at Clinton Mills and one is
out on Leave of Absence, may premiums
for spouse be deducted from
my cnecKY
A. Yes if you will contact Payroll
Department and make arrangements
with them.
Q. If I fail to pay pay premiums
on my insurance while on Leave of
Absence and my insurance is discontinued,
may I be reinstated at a later
date?
A. Yes you may, but you are required
to complete proof of insurability
forms on all members of your
family and effective date and coverage
is determined by Insurance Company.

on my insurance and recently was
married, how do I go about insuring
my spouse?
A. You must take your insurance
policy to the Personnel Department
within 31 days and spouse will be
covered on the following Monday. If
you wait longer than 31 days, insurability
forms must be filled out and
effective date of insurance on spouse
is determined by Insurance C.-mpany.
Q. If I already have dependent
coverage insurance and have an addition
to my family, what should I
do to cover additional child?
A. If yon are already paying dependent
coverage, child will be insured
automatically.
Q. I was employed at Clinton Mills
two months ago and when employed
I took the insurance to cover my
spouse, is there a waiting period of
maternity?
A. If you selected the dependent
coverage when you were employed
